Tower Health Medical Group and Access Services, the county provider of Street Outreach, partnered to establish Pottstown Street Medicine in January 2021. Street Medicine works to provide primary and urgent care to those experiencing homelessness in Pottstown Borough wherever they reside- whether that be a tent in the woods, a homeless shelter, or on the street. Since inception, Pottstown Street Medicine has grown to include additional partners including Community Health and Dental Care and Creative Health Services.

Street Rounds are implemented every Tuesday from 10:00 a.m. to 2:00 p.m. During rounds, the team carries a large backpack filled with medical supplies and is able to provide onsite primary and wound care services. Care coordination is a large component of Street Medicine as the team works to connect patient’s to ongoing primary care and specialty services such as cancer treatment, podiatry, pain management, women’s health, dental care, surgical, and behavioral healthcare services. 

Pottstown Street Medicine works to (1) improve access to healthcare for those experiencing homelessness (2) Reduce unnecessary emergency department visits and hospitalizations (3) Establish trust between medical providers and the homeless community and (4) Bridge the gap to routine primary and behavioral healthcare.

Picnic in the Park

TriCounty Health Council is hosting a Picnic in the Park at Riverfront Park in Pottstown on Tuesday, September 21st from 12pm-1:30pm. Come join Pottstown Hospital, Creative Health Services, and Community Health and Dental Care for a free lunch and learn about substance use prevention, treatment, and local resources. Guest Speaker, Andre Reid, will share his lived experience and dive into substance use disorder and treatment. Andre and his outstanding work were recently featured on the cover of the Philadelphia Metro.

 

This event is completely free and open to all community members and community partners. We hope to see you there!

COVID-19 and Routine Vaccinations: What Parents Need to Know

Join the TriCounty Health Council on Thursday, July 22, at 7 p.m. for a conversation on getting kids back to school safely. 

Expert panelists from around the area will discuss the role of routine vaccinations, COVID-19 vaccines, and the steps we can all take to protect one another and ensure that in-person learning is safe and accessible for kids across our local school districts.

Registration is live now, and the event is free and open to the public. We want your voice to help steer the conversation, so registrants will have the opportunity to submit questions for the panel upon registering, in addition to participating in the live Q&A during the town hall.

Street Medicine Program

Tower Health Medical Group and Access Services, the county provider of Street Outreach, partnered to establish Pottstown Street Medicine in January 2021. Street Medicine works to provide primary and urgent care to those experiencing homelessness in Pottstown Borough wherever they reside- whether that be a tent in the woods, a homeless shelter, or on the street. Since inception, Pottstown Street Medicine has grown to include additional partners including Community Health and Dental Care and Creative Health Services.

Street Rounds are implemented every Tuesday from 10:00 a.m. to 2:00 p.m. During rounds, the team carries a large backpack filled with medical supplies and is able to provide onsite primary and wound care services. Care coordination is a large component of Street Medicine as the team works to connect patient’s to ongoing primary care and specialty services such as cancer treatment, podiatry, pain management, women’s health, dental care, surgical, and behavioral healthcare services.
